CHDFS, Inc. Information

CHDFS Inc. is a not-for-profit organization providing support at home and within the community to children and families under the poverty line and/or going to some difficult times. CHDFS provides services to children enrolled at the NYS Office of Mental Health Home and Community Services in NYC, at the Ofice of Children and Family Services Bridges to Health (B2H) Home and Community Waiver Services for NYC, Westchester and Rockland, to the Office of People with Disabilities Medicaid Service Coordination for Queens, Manhattan and The Bronx, to the ACS - Parents Advocate for Manhattan, Bronx and Staten Island, and recently licensed as an Article 36 Home Care Agency for NYC and Rockland Counties.
