Celere Health is an industry leading platform-as-a-service that integrates in-home healthcare, telehealth and remote patient monitoring (RPM) specifically for the elderly, very young and other specialty populations. There are hundreds of companies trying to bring telehealth and RPM to the healthcare industry, but the vast majority are siloed, independent platforms that neither include a user experience designed for these audiences nor integrate into the workflow of the disparate clinical and care coordination teams - challenges that result in lower utilization and clinical outcomes. Celere Health addresses these challenges with four areas of emphasis: 1. A platform designed to optimize provider workflows, which increases adoption and utilization. A better provider experience ensures better patient experience. 2. A user experience specifically designed for vulnerable populations with low technology adoption and their specific clinical conditions. 3. A hub and spoke technical architecture ensuring easy adoption and quick integration of third-party monitoring devices. 4. Services that fill the staffing gaps of providers and payers to enable 24x7x365 alert management, engagement and response. We know these gaps exist because of our prior professional experience at TeamMD/UnitedHealth Group, where we could not find a market solution that addressed our basic requirements.
