CareLink Tanzania is building a digital teleconsultation and remote patient monitoring platform focused on chronic diseases such as Diabetes and Hypertension. The platform enables patients to remotely log daily vital signs while generating real-time alerts for abnormalities, helping doctors simplify and improve follow-up care. Through CareLink, patients can be continuously monitored by their registered healthcare providers from anywhere, enabling early detection of complications, improved medication adherence, and timely medical interventions. The goal is to reduce uncontrolled Diabetes (DM) and Hypertension (HTN) by making chronic disease management more proactive, accessible, and data-driven. The platform primarily supports high-risk patients who require frequent monitoring, including hypertension patients with uncontrolled blood pressure above 150/100 despite medication adherence, individuals with a history of stroke, heart attacks, or kidney disease, patients with dual diagnoses, elderly patients aged 50+, pregnant women with gestational hypertension or preeclampsia, and patients with poor medication adherence. It also supports diabetes patients with HbA1c levels above 9%, diabetic foot ulcers, neuropathy, diabetic retinopathy, chronic kidney disease, Type 1 diabetes, and frequent hypoglycemia episodes. These conditions often require daily monitoring with 2–3 vital checks per day, making remote patient monitoring essential for preventing severe complications, reducing hospitalizations, and improving long-term patient outcomes.
