Bridges Self-Management Information

Bridges is a social enterprise that exists to make a difference to the lives of people who live with acute and long-term conditions, by working with teams from health, social care and the third sector, to define and deliver best practice in self-management support. We deliver bespoke training courses and consultancy services, informed by our own research and development programme. Co-production is at the heart of what we do; we work with individuals with different health conditions and their families, to produce unique self-management resources that are available as part of our bespoke training packages. We draw on evidence from healthcare policy, research and our own research and development programme, to understand how to integrate effective self-management support in different healthcare contexts, taking into account; the healthcare setting (acute, inpatient, community), different health conditions and the workforce. Our expertise lies in training the health and social care workforce to deliver effective and successful self-management support, sustaining those changes over time and evaluating the impact of self-management support.
