Our direct support professionals are experienced, thoughtful, and well-trained. We are here to create an individualized care plan that is right for you and your family. Bridge To Home recognizes the widespread burden of COVID19. We are acutely aware that people with disabilities or other health issues may be at risk for more serious complications of COVID19*. Remaining healthy is important. Yet, being at home can be isolating. We help people with safe companionship navigate daily activities at home and out in the community. We recognize the enormous impact of chronic caregiving. Balancing work and other family responsibilities with chronic caregiving for a loved one can be challenging. It can lead to exhaustion and little time for self-care. We can help.
