By only screening and intervening on social needs when members show up in a clinic, health plans are unaware of ~95% of the needs that members are experiencing in real time – especially for members who are not already engaged with healthcare services. bosWell flips this approach by transforming community-based organizations (CBOs), such as food pantries, into the hubs for social and health care engagement. Right when members seek help at local CBOs, bosWell’s team of Community Health Advocates engage with these members to offer support – such as warm referrals to other CBOs and nudges towards preventive care.
