Lamprey Health Care is a Community Health Center with three locations in Southern New Hampshire. Our mission is to provide high quality primary medical care and health related services, with an emphasis on prevention and lifestyle management, to all individuals regardless of ability to pay.
Lamprey Health Care’s family physicians are Board Certified and have studied in six broad areas of medicine: pediatrics, surgery, internal medicine, obstetrics and gynecology, psychiatry and community medicine. In addition to medical care, Lamprey offers many other services in response to community needs such as parenting education classes, social services and care coordination, medication assistance programs, and transportation for senior citizens and the disabled in Rockingham County and parts of Strafford County.
Lamprey Health Care is available as a community resource to assist individuals with navigating the new Health Care Market place and New Hampshire's expanded Medicaid Program, the NH Health Protection Program. Assistance with understanding the new health insurance options is available to all individuals in the community.
Fourth Street Clinic was founded in 1988 as a triage clinic; staffed with one part-time nurse who relied heavily on hospitals to treat patients. Today, with a staff of over 100 and a volunteer network of more than 150, Fourth Street Clinic is a federally qualified community health center serving over 6,000 men, women and children experiencing homelessness each year, with over 30,000 total medical, mental health, substance abuse, dental, and case management visits. The ALSAM Foundation Pharmacy at Fourth Street Clinic dispenses 90,000 medications annually.
By increasing access to primary, integrated health care for unhoused Utahn's, Fourth Street Clinic is a major partner in ending homelessness, promoting community health, and achieving across-the-board health care savings.
In 2019, Fourth Street Clinic introduced Mobile Health Care, a mobile clinic serving homeless individuals and families scattered throughout the Salt Lake Valley. This new program gives clients staying at the new homeless resource centers regular access to a doctor, improving their chances of escaping the spiral of homelessness.

South End Community Health Center (SECHC) is a comprehensive health care organization for all residents of the South End and surrounding communities. We are committed to providing the highest quality, coordinated health care that is both culturally and linguistically sensitive to every patient, regardless of ability to pay.
SECHC was founded in 1969 by neighborhood activists in response to health care disparities and lack of access to quality health care in Boston’s South End.
SECHC provides primary medical care, behavioral health services, dental and eye care. We also offer a broad array specialty care services and wellness programs that address the social determinants of health. After nearly 50 years the health center is widely recognized for its outstanding holistic care provided to all, regardless of race, ethnicity, sexual orientation, gender identity, physical ability, and/or income.
Services and Programs
SECHC’s multicultural and multilingual staff of 200+ serves 14,000+ patients accounting for 80,000 annual visits with an operating budget of $18.9 million. Our main clinic, located just steps from the South End’s Blackstone and Franklin Squares at 1601 Washington Street, is a 48,000 square foot state-of-the-art facility.
SECHC offers a broad spectrum high quality services and programs including primary care and prenatal medicine, behavioral health, dental and eye care. Our specialty care services include addiction medicine/office based opioid treatment, civil surgeon/immigration physicals, diabetes management, family planning, HIV/AIDS & HCV testing and care, midwifery, neurology, nutrition, OB/GYN, pediatric asthma/pulmonology, pediatric dermatology, podiatry, sexual health/STIs.
SECHC’s Dr. Gerald Hass Center on Shawmut Ave. is Boston’s first full-time elementary school-based health center offering medical, behavioral and dental services to the local community and students of the Blackstone Innovation Elementary School.
