APNT, Inc (APNT)collaborates with physicians and hospital case-managers to maintain home care as a viable alternative to hospitalization. APNT coordinates cost efficient services with the insurance companies to provide optimum healthcare delivery, specific for each pediatric patient. Our skilled nurses are experienced in providing specialized home care to our pediatric patients. To augment our permanent staff, APNT utilizes the services of independent contractors who specialize in providing care to our clients with high tech needs. Once a decision has been made to utilize the services of APNT, our Director of Nursing, will coordinate with other health care providers and develop a plan of care agreed upon by all health care professionals prior to hospital discharge. APNT's nurse liaison will be in contact with the physician within 24 hours after the child arrives home. An open line of communication is maintained with the physician to ensure the patient receives high quality, cost effective in-home health care services. Our service covers Baltimore City, Anne Arundel, Baltimore, Calvert, Caroline, Carroll, Charles, Dorchester, Frederick, Hartford, Howard, Kent, Montgomery, Prince Georges, Queen Anne's, St. Mary's, Talbot and Washington Counties in Maryland.
