At the Allergy and Asthma Center of Rochester, we are committed to providing the best possible care to each of our patients, as we have for over 40 years. To make your appointment go more smoothly, please read the following:Chart Numbers: At your initial visit, you will be assigned a patient number so that we may provide you with more accurate, efficient service. We ask that you please sign in with this number at each visit and give your number anytime you call our office. This will allow us to have your file at hand should you have any questions. Fees: We ask that all fees for physician visits and allergy testing be paid at the time of service. However, patients receiving frequent appointments may pay on a monthly basis. Payment may be made by cash, check or credit card (Visa, MasterCard, American Express or Discover). For your convenience, we directly bill participating insurance plans. If you have a coinsurance and/or deductible that your insurance company assigns to you, you are responsible for paying the full amount within 21 days or less from the time you receive a bill from us. If you have a flat office visit copay, you must pay this at the time of check-out. Payment plans are available upon request; please ask our friendly staff for more details. Effective January 1, 2015, all new patients must complete a credit card authorization form in order for physician services to be rendered. This form is included with the patient forms packet from our web site download. If you opt to fill out the patient forms in our office, this credit card authorization form will be included. Insurance: We accept most major insurances, including HMO and PPO plans. If you are unsure of your coverage, please call our office, and we'll be happy to let you know if we are a participating provider. For your convenience, we directly bill all insurance plans for which we are a participating provider. However, if we are not a participating provider, please be aware that you, not your insurance, are responsible for fees. You can also find out right away which insurance plans we participate with on our Web site if you click on the "Insurances Accepted" link on the top right of this page. Managed Care: If you are enrolled in one of our participating managed-care plans, you must have a referral from your primary-care physician at the time of your visit. If you are unable to get a referral by your appointment time, please reschedule your appointment for a later date. Referrals are required by your insurance so as to avoid any unnecessary delay in your medical care. Please be sure to have your referral with you at the time of your appointment. If you do not have a referral, please be aware that you will be responsible for payment at the time of your visit. Allergy Shots: All patients are required to wait 15 minutes after receiving their allergy injection(s)in the office as an allergic reaction could occur that requires immediate treatment by the physician. Any patient leaving before the 15 minute waiting period does so at their own risk. Furthermore, patients taking allergy shots who come close to closing should be here no later than 15 minutes before lunch or the end of the day when we close. Prescription Refills: For all prescription refills, please note that we do not call your pharmacy. Instead, call your pharmacy and have the pharmacist call our office for refills. Please note that the law in Michigan prohibits dispensing prescription medications to patients who have not been seen within one year. So, if you have not seen the doctor within a year, please make an appointment to see the doctor. Phone Calls: If you should have a health related concern, please feel free to call our office. Many questions can be answered by our nurses, after consulting with one of the doctors. If you would like to speak to Dr. Ringwald or Dr. Czarnecki, either one of them will be glad to talk to you if your call is urgent. Emergencies: We treat all emergencies in our office. If you feel sick and need to be seen at our office, just call, we will be happy to see you the same day. If you have an emergency after office hours, please go directly to Crittenton Hospital Medical Center, Rochester or Beaumont Hospital, Troy emergency room for treatment. Always call 911 if your emergency is life threatening! HIPAA: Our office is compliant with the U.S. Government's Health Insurance Portability and Accountability Act of 1996 (HIPAA).
