Dynamic Health Systems created VitruCare 365® as an innovative patient engagement platform to be used in healthcare settings that was created to address the challenges of providing personalized care to patients with chronic conditions or other conditions involving complex care plans. The platform was developed by a team of healthcare experts in collaboration with their patients who recognized that engaging patients in their care and motivating them to take an active role in managing their condition has a very positive effect on health outcomes resulting in significant time savings for healthcare professionals and future healthcare costs. With the Platform Dynamic Health Systems assists healthcare service providers to achieve digital transformation in two critical areas: 1) Innovating the way patients take part in their healthcare. We call this PEACH (Patient Engagement and Activation in Co-creating Health) 2) Innovating the way that care professionals collaborate with patients and each other. This we termed CIHWI (Care Integration and Health Work Innovation) This "fruit salad" approach has helped several healthcare service providers to achieve major clinical and operational benefits and win awards in health innovation.

The role of the West of England AHSN is to deliver positive healthcare outcomes in the region and nationally by driving the development and adoption of new innovations and making a meaningful contribution to the economy. The impact of our work is far reaching: from enhancing patient wellbeing, saving lives and mitigating high risk incidents such as strokes; to enhancing efficiencies in healthcare practice and boosting the local economy through private sector collaborations. Our network includes a wide range of partners – health service providers, care professionals, commissioners, service users, the general public, businesses, universities and other research bodies – to act upon healthcare challenges and opportunities for the ultimate benefit of the patient.
