The initial product in AgileCare’s multi-product suite is the AgileCare Log. The AgileCare Log is a customizable daily care routine and wellness tracker. The Care Log manages day-to-day care while providing preventative healthcare insights to keep patients out of the hospital and reduce the overall cost burden on the patient and their families. Through the use of the Care Log’s collaborative portal, families, agencies and healthcare personnel may view the patient’s current wellness and participate in the care of that patient as a social-media-style conversation and not just a clipboard.
