Every patients care plan and authorization are reviewed and the highest Level/Rate is authorized. Case Managers closely monitor each patient stay and are proactive with denial management to ensure authorization through discharge. Case Managers handle all faxing of medical records and updating insurance case managers on patient care plans, changes in statuses, new diagnosis etc. The HMO Case Management process is often fragmented in facilities and we streamline this process and communication to ensure maximized reimbursement and most efficient processes.
