Our mission is to ensure that clients maintain physical health, receive mental health support, and build social connections, fostering a balanced and resilient community. This commitment to holistic wellbeing embodies our vision for a healthier, more connected future. As a registered charity, we offer free and affordable health and community programs to those in need, including: • NDIS Services: We provide Support Coordination, Recovery Coaching, and therapy services. Our expertise in psychosocial disability allows us to offer tailored support to NDIS participants with diverse needs. • Mental Health: Our services include interventions, counseling, and suicide prevention programs. • Healthy Living: We run group and individual programs aimed at preventing chronic diseases and promoting healthy lifestyles. • Condition Support: We offer management, support, and education for various chronic conditions. • Street Doctor: As Perth’s original mobile GP and outreach service, we serve the homeless and disenfranchised. Our ultimate goal is to help build a stronger, more collaborative health system that connects the health and social sectors, supports smaller community organisations, and helps keep people out of hospitals. We believe in collaboration over competition and are always seeking partnerships with like-minded organisations.
