NexusDocs, a division of Heart Nexus, is an experienced team of board certified physicians providing patient care remotely using the latest telemedicine technology and equipment. Our role is to complement existing providers and staff, providing services at places and times most needed to positively impact quality outcomes for the patients. Quality outcomes are defined as: • Maximizing the health status of patient • Reduction in avoidable ER visits and hospital admissions • Reduction in readmissions • Positive facility recognition by patients, payors, regulators and the community WHAT WE CAN PROVIDE: • Availability of physicians specializing in: - Internal Medicine - Cardiology - Pulmonology • Availability of behavioral health providers • After-hours service coverage from 6PM to 6AM daily (Monday – Friday), weekends and holidays - Response time within 15 minutes of request • Regular hours coverage for appointments - Appointments scheduled within 24 hours • Telemedicine Equipment (both hardware and software) • 24/7/365 technology support • 24/7/365 communications center • Provider documentation and EMR interface • Participation in quality improvement initiatives and programs • Facility onboarding and implementation • On-going performance review and improvement - Monthly performance review and discussion - Monthly service reports - Review of service enhancement opportunities • Provision and interpretation of on-site cardiac, vascular and other diagnostic testing
